PROCESS CHART IN SECURING BUSINESS LICENSE AND PERMIT
START
  • Process 1. Business Applicant to submit Barangay Clearance to the License & Fees Division (at Window 9 or 10).
  • Process 2. License & Fees Division Personnel gives application form forCommunity Tax Certificate (Cedula) to the business applicant and applicant fills-up the form.
  • Process 3. License & Fees Division personnel assesses/computes the CTC (Cedula) application form.
  • Process 4. Business applicant submits CTC (Cedula) application form already assessed/computed to the Revenue Collection Officer/Clerk at (Window 1 to 7) for the issuance of CTC (Cedula).
  • Process 5. License & Fees Division personnel gives blank Application Form for Business License & Permit (with built-in clearances, checklist at the back thereof) and applicant fills-up the form.
  • Process 6. Business applicant handcarries duly filled-up Application Form for Business License & Permit for the signatures of the following:
    1. Chief, Real Property Tax Division,
    2. Market Supervisor IV, (for stallholders only)
    3. City Planning & Development Officer,
    4. City Health Officer,
    5. City Engineer,
    6. City Fire Marshall. (NOTE: All with built-in clearances in the form)
  • Process 7. Business applicant handcarries duly filled-up Application Form for Business License & Permit to the License & Fees Division for assessment/computation of the business tax, fees & other charges at (at Window 9 or 10).
  • Process 8. License & Fees Division personnel assesses/computes the tax, fees & other charges using the Business Assessment Form, then the Chief, License & Fees Division signs the application form (see built-in clearance).
  • Process 9. Business applicant handcarries Business Assessment Form together with the duly filled-up Application Form for Business License & Permit to the Revenue Collection Officer/Clerk (at Window 4 to 7).
  • Process 10. Revenue Collection Officer/Clerk issues Official Receipt/s and indicates the amount paid, O.R. No. and date of payment on the application form and affixed his signature thereon (at bottom portion or details of payment in the form).
  • Process 11. Business Applicant handcarries Application Form for Business License & Permit for the signature of the City Treasurer.
  • Process 12. Business Applicant handcarries duly filled-up Application Form for Business License & Permit to the following offices for clearance purposes:
    1. PNP (100 meters away more or less from the City Hall),
    2. BIR,
    3. SSS,
    4. PHILHEALTH,
    5. DTI, &
    6. PAG-Ibig Fund.
  • Process 13. Business Applicant handcarries duly filled-up Application Form for Business License & Permit to the Office of the City Mayor for the issuance of Mayor’s Business Permit.
  • Process 14.The Community Affairs Officer signs & releases the approved Mayor’s Business Permit.
